You can access each application form in a PDF formatted file.  There are four applications defined at this time:  Corporate Membership, Certified MBE, Recertification and Affiliate Reciprocal Certification. 

Corporate Membership application is for companies who support the activities of the Florida Minority Supplier Development Council, Inc.   There are 5 levels of support:  Gold, Silver, Bronze, Basic and Government.    Corporate Questions

Minority Business Enterprise (MBE) application is for certification of businesses owned at least 51% by Asian-Indian, Asian-Pacific, Black, Hispanic and Native American. For further definition of MBE, click here.

Affiliate application/Reciprocal is for businesses that are certified by an affiliated council of NMSDC, Inc.

Award Applications the following are the applications for the annual corporate and MBE awards. Only Corporate Members of FMSDC and MBEs certified by FMSDC are eligible for consideration.
